Be Well Memory and Infusion services are provided through a unique concierge model, with various level options. No matter the level of care you choose, patients and their caregivers will benefit from leading edge technology and the latest in evidence-based therapies.

Three Levels of Service – To Meet Your Needs

For any level of service, all patients receive unlimited access to our clinical team.

Choose a package that best suits your needs. As your health changes, the flexibility of our levels will allow you to modify your plan as you desire. We are with you every step of the way.

Be Well Health Assessment

Our services start with a comprehensive health assessment where we take time to get to know each patient on a personal level. We will review your medical history, current lifestyle, personal health and cognitive status.

Then together we will develop a care plan that is specific to you and meets your health goals.

Early Detection Means Early Intervention

Be Well MD now offers Independent Care Support, a state-of-the-art continuous assessment technology, to raise our standard of care and ensure the safety and well-being of our members.

INDEPENDENT CARE SUPPORT – HOW IT WORKS

Easy-to-use plug and play audio pods gather care-related data around the clock with the goal to:

Detect

Data-gathering for early detection of care issues that may require attention.

Predict

Information gathered will be aggregated into a Be Well MD Care Report that enables early intervention to resolve care-related issues.

Prevent

Identifying patterns of activity that may lead to more serious problems.
The easy-to-understand Be Well MD Care Report contains insights and recommendations on the data gathered from Sensi and are sent X times per week with full transparency to all care-related occurrences found.

These Care Reports will include highlights of issues or concerns, insights and recommendations from the data with a timeline of events, notable events, and our recommended next steps. The Be Well MD Care Report can also be sent to others on the care team, as preferred.

Medical Alerts

If we receive a non-urgent notification that is medical in nature, we’ll move up the next appointment or we’ll address it during the next regularly scheduled appointment time. These alerts will be included in the Be Well MD Care Report. Be Well MD will address to any urgent notifications in the event of a fall, cry for help or EMS.

Non-Medical and Non-Urgent Medical Alerts

All other notifications will be communicated in the Be Well MD Care Report delivered X times per month. It is up to the member or care providers to take action on these issues.

Independent Care Support augments a member’s unique in-home care plan to offer a 24-hour safety net, helping to:

  • Reduce hospitalization risk

  • Identify symptoms of infection or decline early on

  • Inform family for those disinclined to proactively seek help for problems

Focused, Secure and Private

  • We take privacy seriously!

  • Data gathering is auditory only (no cameras) and focused on care-related activities.

  • No “live” monitoring is enabled or involved, data is sent to Be Well MD.

  • Data privacy practices comply with HIPAA and ISO27001.
